Where can I get the Consent Forms?

We have a Disclosure and Authorization template if your organization doesn't have consent forms but we recommend checking with your legal counsel.

The forms will be located on the Home Page of your account in the Compliance Documentation card.

• Click the "View Forms and Documents" link and then select "Standard FCRA Disclosure and Authorization Form.

• You should provide your applicants with the "A Summary of Your Rights Under the Fair Credit Reporting Act" document as well which is also located here.

If you use our Online Consent Form (available with the Bundle 2 subscription and higher), the Summary of Your Rights document will be available to the applicant through the online form.
