Creating a New Form

The Bundle 4 subscription allows you to add additional Forms. You can add different Custom Questions onto these new forms, if you need to ask different questions for a certain Position. 

Create a New Form 

  • Click on the Settings tab in the lefthand menu
  • Click on the "Departments" option under the Settings tab

  • There will be a blue "New" link to the right side of the page

  • Name - Name of the Form
    • Your applicants will not be able to see the name of the Form
  • Authorization Required - You are able to turn off Authorization, which will remove the last page of the form, where the applicant give their consent to have a background check ran on them. If Authorization is turned off, you are able to edit what information is collected on the form
    • Note: You will not be able to order a background check on applicants if Authorization is not collected
  •  Authorization Text - This will be the text that your applicants see when signing their consent to have a background check ran on them. When you create a new form, our default text will populate, but you are able to customize the text
    • Note: Our text has been approved by our legal team. You may want to contact your legal team to make sure that any changes you make are in compliant

Applicant Information:

Below the Authorization Texts are checkboxes that will require information on the Online Form.

When the Authorization Required checkbox is on, the Name, Other names used, Address, SSN, Date of Birth, and Email checkboxes are automatically turned on, and cannot be taken off. 

When the Authorization Required checkbox is off, only the Name and Date of Birth cannot be taken off.

  • Name - Requires the applicants to enter their full name.
    • This option cannot be turned off
  • Other names used- Allows applicants
  • Address - Requires the applicants to enter in an address
    • If the address is 4 years or under, the form will require the applicant to enter in another older address
  • SSN - Requires the applicants to enter in their Social Security Number
  • Date of Birth - Requires applicants to enter in date of birth
    • This option cannot be turned off
  • Phone Number - Requires applicants to enter in their phone number
  • Email - Requires applicants to enter in their email address and to confirm the email address
  • Require Drivers License - Requires the applicant to enter in their Driver's License number and Driver's License state issued.
    • This is not automatically turned on
  • Gender - Requires applicants to choose their gender
  • Ethnicity - Requires applicants to choose their Ethnicity

 

Stock Questions:

Right below the Applicant Information checkboxes will be another set of checkboxes. These are prepopulated questions that you can add onto your Online Forms.

  • Ask Education Questions? - Asks applicants to list the highest academic information.
    • If ordering the Education Verification, this information will automatically fill out the Education Verification order screen
  • Ask Employment Questions? - Asks applicants to list previous employment information
    • If ordering the Employment Verification, this information will automatically fill out the Employment Verification order screen. 
  • Ask Volunteer Questions? - List of organizations where the applicant has volunteered at
  • Ask DMV Questions? - Asks applicants if they've had any traffic violations or accidents
  • Ask Children Questions? - Asks applicants if they've had anything in their past or present that would prohibit them from working with children and/or students
  • Ask Custom Stock Questions? - Ask Custom Questions on this form in addition to any Custom Questions set up on this form

Preview Tab

The Preview Tab will show you how your questions and Authorization Text will look when you send out the Online Consent Form to your applicants. You can click on the "Next" button on the bottom of the page without needing to enter in any information.

  • The first page will always be where the applicants will enter in their information
  • The last page will be the Authorization page, where the applicants give their consent (unless the Authorization Required checkbox is turned off)
  • If there are any Custom or Stock Questions, there will be a page in between the Applicant Information page and the Authorization page.
